Dyspareunia: The pain is physical and emotional

When the members of Vibrant Nation come together to talk about sex — and they certainly do — we often hear about a member who’s having a specific problem in the bedroom. Because Vibrant Nation is a site for women 50 or older, often that problem has a name: dyspareunia.

Dyspareunia is, literally, “painful sex.” After 50, sex can be painful due to female dryness, deterioration of vaginal tissues, and many other reasons. Member mostlikely2 knows all about the pain of dyspareunia and the heartache it can cause:

“Sex has become a dread..it just plain hurts,” she shared. “I have tried a lot of creams and inserts and the results have been mixed. I now approach sex with anticipation of discomfort and THAT affects everything. I’ve become very creative in avoiding it altogether – much to the chagrin of my husband. The saddest part is I used to LOVE it…I miss loving it!!!”

Member Snow has had a similar bout with dyspareunia:

“Due to loss of estrogen and wall thinning, sex has become excruciating over the last few years,” she said. “I have been to numerous doctors, had surgery, spent thousands of dollars and get temporary relief with each new treatment that eventually doesn’t work anymore. My newest expert from University of Miami has me using Premarin cream, which is working.”

According to Dr. Sheryl Kingsberg, “there is only one FDA approved pharmacologic agent for women with a sexual disorder and that is Premarin Vaginal Cream for the treatment of moderate to severe dyspareunia.”

Dr. Kingsberg said that often, women’s sexual problems overlap. For example, if a woman’s primary sexual problem is dyspareunia — pain as result of decreased estrogen and subsequent vulvovaginal atrophy — she may also now experience decreased desire. After all, who has interest in an activity that causes pain? Subsequently, arousal is decreased, “and with all that, you can forget about achieving orgasm.”

There are many causes and solutions for female dryness and dyspareunia, so it’s vital that you talk to a medical professional about these most intimate physical issues. If you prepare yourself for the conversation you’ll feel less apprehensive and you’ll be able to work with your doctor to choose the best course of action.

In most cases, your doctor will examine you to find any possible physical causes for dyspareunia. (Dyspareunia may also be caused by psychological or emotional issues.) In the majority of instances of dyspareunia, there is a physical cause. An extreme form of dyspareunia is vaginismus, where the pelvic floor musculature involuntarily contracts.
